Please provide us any pertinent reports (eg, IEP's, neuropsychologist reports, previous speech/language reports)
Communication Evaluations: Consider bringing a snack, book, or preferred toy for younger children.
Orofacial Myofunctional Evaluations: Please bring in a drink and few snacks of choice.
Please print off and complete the forms below and present them to your clinician upon the arrival of your session. If services will be provided through telepractice, the telepractice consent must be completed.
We are out-of-network providers for insurance companies and we are not responsible for submitting claims. It is the client’s responsibility to inquire on benefits and coverage. If a client wishes to submit a member claim to an insurance company, we will provide you a superbill/account statement and assist in providing an additional paperwork as needed.
